CBT in a family setting is based on the belief that the behaviors and thoughts of a single member of the family can considerably propel the progression of family dysfunction or prevent adjustment in the system. CBT techniques are used in a family setting to help family members with solving family conflicts that lead to problems and dysfunction in the family system (Day et al, 2017). For instance, CBT can be for children and parents who have physically abused their children. The goal is to assist children who may have had abusive experiences get an opportunity to heal, empower parents on how to deal with their children well in a non-coercive way, strengthen relationships between parents and children and enhance the safety of every family member.

CBT in a group setting focuses on assisting a group of individual deal with a common maladaptive thought or behavior. Bjorsson et al (2011) note that in a group setting patients learn how to recognize and interrogate maladaptive thought and take part in exposures to situations they are afraid of to test examine the correctness of biased beliefs. For instance, cognitive behavioral therapy can be used in a group of people with social anxiety disorder. Feared stimulus like public speaking is recreated in a group setting to permit individuals to adjust to their fears.

Counselors might encounter a challenge of matching each client with the suitable group when using CBT. Prior to placing an individual in a certain group, the counselor must consider the needs, preferences, and characteristics of the client. Center for Substance Abuse Treatment (2010), argues that individuals might need to shift to different groups as treatment progresses, encounter obstacles and become less devoted to recovery. The other challenge is dealing with people of diverse cultures. Cultural practices might influence communication among members of the group.
